Key Responsibilities
The Hospice Social Worker will be responsible for assisting patients and families in managing their needs related to end of life issues. This includes:
- Completing admission and transfer process.
- Providing counseling and education on psychological, emotional and social aspects of death and dying.
- Assisting patients and families in identifying and utilizing appropriate resources.
Requirements
- Master's degree in social work.
- Current Michigan Social Work licensure.
- Sound clinical judgment and ability to work independently.
- Knowledge of and/or experience with end of life care preferred.
