Position Overview:
The Case Manager is responsible for assessing the needs of hospice patients and coordinating their care with the interdisciplinary team. The Case Manager provides direct nursing services in the patient's home or place of residence.
Key Responsibilities:
- Conduct initial and ongoing assessments of patients' physical, psychological, social, and spiritual needs to develop and implement individualized care plans.
- Manage and coordinate all aspects of patient care, including medication management, pain and symptom control, and emotional and spiritual support.
- Collaborate with physicians, nurses, social workers, chaplains, and other members of the interdisciplinary team to ensure seamless and coordinated care.
- Educate patients and families about hospice services, disease processes, and end of life care options.
- Maintain accurate and up-to-date documentation of patient care, including assessments, interventions, and outcomes.
- Adhere to all hospice regulations, policies, and procedures.
- Advocate for patients' rights and ensure that their wishes are respected.
- Participate in quality improvement initiatives to enhance the quality of hospice care.
Qualifications
Required Education/Credentials:
- Graduate of an accredited nursing program.
- Current California Registered Nurse license.
Required Experience:
- Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program.
Required Skills and Abilities:
- Strong clinical assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Ability to manage a caseload effectively and efficiently.
- Compassionate and patient-centered approach to care.
- Proficiency in electronic medical record (EMR) systems and other workplace technology.
- Valid driver's license and reliable transportation.
Preferred Qualifications:
- Experience in hospice or home health.
- Experience working effectively with an interdisciplinary team.
- CHPN certification.
Salary : $45 - $55
