What are the responsibilities and job description for the LPN, Crisis Care Team position at Hospice of Lake Cumberland?
Hospice of Lake Cumberland is looking to hire a LPN for our Crisis Care team. This position will be a swing shift position with 2 dayshifts and 1 nightshift per week. Hours will be from 8 am-8 pm for dayshift and 8 pm-8 am for nightshift. The schedule will include a weekend rotation. Total hours will be 36 hours plus drive time from a designated Hospice office.
Job duties will include one on one patient care in the home setting; teaching patients & families about end of life care and the needs of the patient; and collaboration with case manager regarding symptom management of the patient.
