At Cornerstone Hospice, the Clinician/Admissions role is responsible for determining the eligibility and preparation of patient/families for hospice care. This can be done in Hospitals, ALF or LTC facilities, and/or the home environment throughout Lake and Sumter Counties. This position is responsible for all activities designated to enhance and coordinate relationships with hospital referral sources, increase referrals, and assist in timely admissions.
SCHEDULE: 4 Days - 10 hour shifts 9AM-7PM - rotating weekends (Lake and Sumter County)

Position Responsibilities:
Develops and maintains quality, professional relationships with referral sources, working professionally with physicians, case managers and staff while representing the interests of Cornerstone Hospice.
Provides information such as updates and educational materials to referral sources, answers questions, resolves problems and promotes Cornerstone Hospice services.
Follows up with manager or designee on issues raised by referral sources.
Provides Hospice information to patients/families, prepares them for admission, initiates the admission process, including principles of effective symptom management, and completes admission paperwork including verbal and written explanation of Patient Rights and Responsibilities, and any other information or consents required by regulation.
Uses established guidelines to evaluate referred patients for Hospice appropriateness, through physical assessment, review of medical documentation including History & Physical, physician notes, diagnostic testing, lab tests and other relevant information.
Provides discharge planning for patients within the hospital setting and initiates prescriptions, medical equipment, staff visits and crisis care as warranted.
Meets the needs of the newly referred patient/families, completing the admissions process as soon as possible after referral; within 24 hours or sooner.
Schedules assessment visit with patient/family, coordinating with the admissions social worker when necessary.
Gives a comprehensive explanation of hospice services and benefits addressing all components of care.
Obtains signatures on all necessary consents ensuring that patient/families are informed of their rights and responsibilities.
Performs a physical/environmental assessment, in addition to a psycho-social and spiritual assessment if an admissions social worker is not involved.
Provides primary clinical care and teaching, as needed, to stabilize the patient and facilitate the admissions process.
Documents assessments and establishes the written initial plan of care and plan of treatment.
Qualifications
- Florida R.N. license required; BSN, Master’s degree or CHPN preferred.
- Three years’ field experience or two years’ Hospice experience or equivalent.
- Excellent oral and written skills.
- Excellent assessment skills with demonstrated ability in physical assessments.
- Understanding of the dynamics of human behavior and the ability to communicate with people in a crisis.
- Personable, strong initiative and ability to work independently.
- Previous teaching/sales experience a plus.
- Must be able to work flexible hours.
- Valid Florida driver’s license and required auto liability insurance required.
