Hospice of Marion County, a member of Empath Health, is seeking a Psychosocial Team Leader. As a Psychosocial Team Leader, you will play a vital role in ensuring that patients receive holistic and comprehensive care that addresses their emotional, social and spiritual needs.
Job Title: Psychosocial Team Leader (PSTL)
Schedule: Full-Time, M-F 8a-5p
Key Responsibilities:
- Supervise psychosocial care, including admissions, transfers, and discharges.
- Provide clinical supervision and mentor new team members.
- Facilitate quarterly bereavement groups and community education on hospice care.
- Address customer satisfaction issues and implement service recovery.
- Lead team meetings, support change management, and participate in on-call rotation.
Qualifications:
- Master’s degree in social work and LCSW certification.
- Three years of medical social work or related experience; hospice experience preferred.
- Valid Florida driver’s license and reliable transportation.
- Typing speed of 25 wpm preferred; must pass computer literacy evaluation.
