RN Case Manager Support: Part Time - 24 hours per week, with a weekend rotation
SUMMARY OF JOB RESPONSIBILITIES
QUALIFICATIONS
- Is a graduate of an accredited school of Nursing.
- Current state of Michigan Registered Nurse License.
- Certification in hospice and palliative care preferred.
- Possesses and maintains a current CPR certification.
- Current Driver’s license.
SUMMARY OF JOB RESPONSIBILITIES
The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice.

ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned)
- Knowledge of Hospice concept and philosophy.
- Observes and assesses patient/family/caregiver and develops plan of care.
- Medication for patient assessed for need and effectiveness on an ongoing basis using comprehensive physical assessment.
- Incorporates safety and teaching into Plan of Care.
- Individual as well as Group consultation to keep Team advised of patient status.
- Assesses patient comfort level at every visit.
- Performs venipunctures lab draws, ports and IV starts as needed.
- Has ability to use IV and enteral feeding pumps; operations and maintenance on meeting and anticipating the needs of patients.
- Works as a team member and collaborates with appropriate co-workers.
- Demonstrates proper hand washing, asepsis, bag technique and universal precautions.
- Facilitates hospice referrals by physicians and hospital discharge planners by liaison activity at area hospitals as needed.
- Assesses and documents appropriateness for hospice at initial and ongoing assessment visit.
- Conducts initial nursing assessment visit and arranges for services to begin.
- Consults with Hospice Physician and/or clinical manager on anticipate needs for new admissions
- Provides information on hospice care and programs to referral sources and patients and families
- Assesses patient/family interest in hospice care and services as needed.
- Determines payer source and coordinates the determination of hospice benefits for the referred patient as needed
- Coordinates with discharge planners to assure continuity of care and patient admission to the hospice program as needed.
- Completes written admission documentation with patients and families as needed
- Provides direct nursing care to patients in order to maintain the highest level of comfort and quality of life. Assists in directing and coordinating the physical care of the patient including providing medication, diet and other instructions as ordered by the physician, Primary Care Nurse, and Clinical Manager during telephone contacts or home visits.
- Supervises and assists in directing ancillary personnel including LPN’s, CHHA’s and Homemakers as necessary. Delegates responsibilities when required.
- Seeks and obtains sufficient information necessary to function effectively in the Hospice setting where patient/family/caregiver require care.
- Provides professional nursing care by utilizing all elements of the nursing process.
- Applies appropriate preventative and therapeutic nursing procedures and techniques and maintains communication regarding patient care with the physician and supervisory personnel.
- Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Delivers all related paperwork to the Hospice office by 10:00 AM the next day. This includes keeping the on call log book current.
- Arranges for equipment and materials needed from community resources. Uses supplies and equipment effectively and efficiently.
- Teaches the patient/family/caregiver self-care techniques as appropriate. Recognizes and utilizes opportunities for health counseling with patients/families/caregivers. Works in conjunction with the other Hospice interdisciplinary team members.
- Performs within the limits of preparation and experience.
- Fulfills the obligation of requested and/or accepted case assignments.
Qualifications
QUALIFICATIONS
- Registered Nurse with degree from an accredited nursing school and holds a current license in the State of Michigan.
- Possesses and maintains a current CPR certification.
- Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
