Social Work & Counseling Support Coordinator
Part Time, 24 hours per week with rotating weekends
SUMMARY OF JOB RESPONSIBILITIES: This position includes a variety of administrative support functions for the social work and counseling departments. These functions are related to Medicaid applications and insurance applications/renewals, patient/bereaved assessments and phone calls and other resources needed to provide administrative support. This position will provide back-up administrative tasks for the intake/referral office functions as needed.
MINIMUM QUALIFICATIONS: Preferred Bachelor’s Degree in Social Work with appropriate state licensure. Possess a comprehensive knowledge of the local community and the ability to coordinate necessary referrals. Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Work requires ability to communicate orally and to hear or communicate with focus population. Bi-lingual skills to work with patients in their native language are preferred; certification as a medical interpreter/translator is a plus. Training in assisting in submission of DHHS Medicaid applications. Preferred training in facilitating Advance Care Plan discussion. Must be comfortable working and serving in a diverse and inclusive environment and operating in a collaborative environment. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Harbor Hospice.
