We are seeking a dedicated and skilled Registered Nurse Case Manager to join our healthcare team. Responsibilities
- Conduct comprehensive patient assessments to develop individualized care plans.
- Coordinate with multidisciplinary teams to ensure effective patient management.
- Manage medical records and documentation accurately and efficiently.
- Provide case management for patients in acute care settings, including Level I and Level II Trauma Centers.
- Oversee the care of patients with complex needs, including geriatrics and infectious disease care.
- Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
- Educate patients and their families about health conditions, treatment options, and self-care strategies.
- Advocate for patients' needs within the healthcare system to ensure they receive appropriate services.
Experience
- Current RN license with relevant experience in case management preferred.
- Previous experience in acute care settings is highly desirable.
- Familiarity with medical records systems and documentation practices.
- Knowledge of emergency medicine practices is a plus.
- Experience working with diverse populations, particularly in geriatrics or infectious disease care.
