What We Are Looking For
Job Title: On Call RN
Schedule: Variable | 8pm - 8:30am | 12/36 or 12/40 hours
Pay structure: Flat on call rate plus hourly rate for visits. This position is eligible for mileage reimbursement
A vital component of this position is communication and collaboration with all members of the care team to ensure a seamless transition/hand off between shifts for the purpose of continuity of care. This position will be on-call to ensure we provide the best care possible 24/7 to each of our patients as we experience an increasing patient census and growing need for hospice services within the community.
- Graduate of an accredited school of Nursing with current and valid NYS Registered Nurse license is required; BSN preferred
- 3-5 years recent clinical nursing experience with minimum 2 years medical surgical experience in an institutional setting, home care or skilled nursing facility preferred
- Hospice admission and end-of-life care experience preferred
- Must have excellent written and verbal communication skills
- Proficient in MS Office Suite, EMRs and ability to learn new software programs
- Must have daily access to a vehicle, valid driver's license, acceptable driving record, and auto insurance to travel to patient locations
- Commitment to the HOSC mission and philosophy of care
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Able to communicate effectively, while sustaining a caring, kind, and professional disposition
- Communicates with Hospice Interdisciplinary Team members on a timely basis regarding changes in patient’s condition that occurs during their shift.
- Provide timely visits to home, hospital or skilled nursing facility in any part of Orange/Sullivan counties anytime during assigned shift.
