What are we looking for?
The RN Case Manager is essential to the Hospice organization. From admission to the end of our patients’ journeys, our RN Case Managers provide professional nursing care by utilizing all elements of the nursing process. We are looking for RN Case Managers to join our field team as leaders in our local community. Our RN Case Manager leads a designated team that includes an LPN and Hospice Aide Specialist who are assigned to their own personal territory located throughout Orange and Sullivan Counties. In addition to providing hands on medical care, the RN Case Managers collaborate with the Social Work and Spiritual Care teams to develop a comprehensive plan of care.
Schedule: Monday through Friday 8:30am - 4:30pm. This position also requires a weeknight on-call rotation.
Who are we looking for?
- Graduate of an accredited school of Nursing with current and valid NYS Registered Nurse license is required; BSN preferred
- 3-5 years recent clinical nursing experience with minimum 2 years medical surgical experience in an institutional setting, home care or skilled nursing facility preferred
- Hospice admission and end-of-life care experience preferred
- Commits to take and pass the hospice and palliative care nursing certification exam (CHPN) within three years of hire if not currently certified
- Must have excellent written and verbal communication skills
- Proficient in MS Office Suite, EMRs and ability to learn new software programs
- Must have daily access to a vehicle, valid driver's license, acceptable driving record, and auto insurance to travel to patient locations

- Ability to relate to a diverse population and workforce in a personable and effective manner
- Able to communicate effectively, while sustaining a caring, kind, and professional disposition

Pay: $87,000.00 - $105,000.00 per year
