Job Title: Social Worker (Per-Diem)
Schedule: Sunday-Monday 8:30am-4:30pm
Location: 800 Stony Brook Court Newburgh, NY 12550
Salary Range: $28.18-$32.00 per hour.
Salary Range is commensurate with experience.
Job Summary:
Responsibilities include the introduction and establishment of Hospice Social Work services with patients and families. This position requires a comprehensive understanding and ability to educate patient and families regarding the Hospice philosophy and services; meeting with patients and families within their homes; completing full psychosocial assessments, providing clinical counseling interventions and case management skills; coordinating with team members on the development and implementation of a plan of care; and providing bereavement counseling.
Essential Job Functions/Responsibilities:
- Provide an assessment within the pt’s identified residence. Assesses the psychosocial status of patients/families/caregivers related to the patient's terminal illness and living environment. Assessment includes Pt’s primary goals and priorities, Emotional status, Advance directives, Cultural considerations, Funeral plans, Bereavement Risk Level, Veteran status and personal needs.
- Assess Veteran status and any evidence of moral injury, PTSD, suicide ideation.
- Creates a safety plan with the pt/family/team to address all mental health and physical needs.
- Creates a psychosocial care plan, performs intervention based on evaluation findings. Counsel's patient/family/caregivers as needed in relationship to stress and other identified coping difficulties related to the patient’s dying process.
- Maintains clinical records on all assigned patients as per agency policy.
- Provides information and referral on community services for Hospice patients/families/caregivers regarding practical and environmental needs.
- Serves as liaison between patients/families/caregivers and community agencies.
- Maintains collaborative relationships with Hospice personnel to support patient care.
- Participates in the development of the Plan of Care and attends interdisciplinary team case conferences weekly.
- Assists physicians and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
- Participates in discharge planning when needed. Assists patient/family/caregiver with Advance Directives and funeral arrangements, as needed.
- Advances professional knowledge and skills.
- Other duties as delegated by the Director of Social Work & Bereavement Services in conjunction with the Vice President/COO.
- Assists in the orientation of new clinical staff.
- Supervises or assists in the supervision of student interns.
- Assists in volunteer training, as needed.
- Supervises volunteers assigned to patients under social workers' care.
- Attends monthly Social Work team meetings, and bimonthly Clinical Peer supervisions.
- Provides clinical service to Bereavement program including phone calls, facilitation of support groups, and individual/family sessions.
- Other duties as assigned
