JOB TITLE: Hospice Aide
RESPONSIBLE TO: Patient Care Manager
POSITION SUMMARY: As a member of the interdisciplinary team, the hospice aide provides personal care services to the terminally ill patient and performs related tasks in the patient/family home in accordance with the plan of care as assigned by the primary nurse. Provides temporary respite and relief from total care responsibilities for the primary caregiver.
ESSENTIAL JOB FUNCTIONS:
- Provide personal care to the patient in a compassionate and understanding manner consistent with the hospice philosophy.
- Under the written and/or verbal direction of the RN, the HA performs the following duties:
- Measure and record vital signs including, temperature, pulse, respirations, and oxygen saturation.
- Turn oxygen off and on.
- Provide bed bath, shower, or tub bath as appropriate.
- Provide appropriate care of skin, hair, mouth, and feet (excluding nail care); nail care for hands will be provided only as directed by the PCM.
- Provide good body alignment, positioning, and range of motion exercises for non-ambulatory patients.
- Assist with mobility of patient, crutches, walker, wheelchair, Hoyer, and sit to stand.
- Assist patient with catheter care, application, and removal of a catheter as well as use of bedpan, urinal, commode, and toileting.
- Assist with ostomy care of an established ostomy, all according to the plan of care.
- Assist with feeding and dressing patients.
- Report any significant changes in the patient’s condition to the RN immediately.
- Change simple dressings.
- Perform various housekeeping duties related to patient care.
- Perform simple procedures as an extension of therapeutic services as delegated by the RN and in accordance with the plan of care.
- Order and maintain supplies as assigned.
- Responsible for maintenance of own entries in to paper and electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards. Participate in ongoing filing and maintenance of the paper medical record as directed.
- Identify and address comfort care needs within scope of Hospice Aide role.
- Support and instruct the patient, family, and caregiver.
- Maintain the dignity of the dying patient.
- Support the patients and family's unique spiritual and cultural beliefs.
- Consult and collaborate with the interdisciplinary team and others involved in the patient’s care, including attending Interdisciplinary Team Meetings.
- Schedule and complete patient visits as assigned by the Patient Care Manager or RN Case Manager.
Qualifications and Education requirements
- Candidate must be a graduate of a state certified Nurse's Aide training program have active Board of Nursing current, active certification on the Oregon Nursing Assistant Registry.
- Previous experience working in hospice, home health or long-term care preferred.
- Must be able to work independently and have strong written and verbal communication skills.
- Computer proficiency and a working knowledge of Microsoft programs required.
- Valid Driver’s License and daily access to a reliable and insured vehicle.
- Proof of COVID Vaccination required per State of Oregon mandate.
