JOB TITLE:                          Registered Nurse Case Manager

 

RESPONSIBLE TO:            Patient Care Manager/Executive Director


POSITION SUMMARY:    Under the overall direction of the Medical Director and the IDT (interdisciplinary team), the Primary Nurse is responsible for identifying and coordinating patient/family Hospice care needs, for preventative and supportive care to terminally ill patients and their families.  The Primary Nurse endeavors to utilize her teaching, assessment, observation and intervention skills to provide palliative care and maximize quality of life to the patient and his/her family.

 

ESSENTIAL FUNCTIONS:


  • At the request of the Patient Care Manager, completes the initial assessment of care needs for patient and family including physical, emotional and mental status. Assesses, when indicated, cultural, religious, motivational, physical, cognitive, language or emotional challenges to provision of care, and plans for appropriate accommodation.
  • Provides education to patient and family consistent with their learning needs, abilities and readiness related to the patient’s health care needs, plan of care, and family’s coping styles and abilities.
  • Follows through and modifies the initial care plan for each patient as the needs of the patient/family change, under the direction of the IDT.
  • Regularly attends and participates in patient care conferences. Attends staff support meetings and staff meetings.
  • Maintains availability for on-call visits to assist and support patient/family through physical and/or emotional crisis situations, 24 hours a day, 7 days a week, on a rotating basis. (Typical on-call schedule is one night per week and one weekend per month.)
  • Offers optimal symptom management, emotional support, and energy-saving techniques to patient and family. Offers support as needed for caregiver(s) so they may keep the patient at home or in the home setting as long as is desired.
  • Conducts weekly on-site evaluations and supervision of the care provided by the Hospice Aides, including review of HA documentation and HA care plan. Completes annual competency checklists for Hospice Aides. Provides on-site evaluation of tasks accomplished by the Homemakers every month as indicated.
  • Communicates with the physicians, Patient Care Manager and IDT concerning patient care as appropriate and documents communication in the patient record or care plan.
  • Maintains up-to-date patient records so that problems, actions and goals are accurate and clearly stated.
  • Performs nursing skills according to standards accepted within the State of Oregon.
  • Exemplifies familiarity of policies and philosophy of Hospice, rules and regulations of State and Federal bodies which aid in determining policies and standards for performance improvements and competencies.
  • Attends conferences/seminars offered by Hospice of Redmond either internally or externally in order to enhance competency and skill. Attends required in-service education.
  • Delegates appropriate tasks to Hospice Aide as needed.
  • Requests referral to other disciplines as needed to enhance team effort and quality of care for patient and family.
  • Updates volunteers as needed with patient’s condition and/or delegates this to Volunteer Coordinator.
  • Coordinates transfer and/or discharge of patient under the direction of IDT/Patient Care Manager, mindful of coordinating plan of care in accordance with policy for continuum of care.
  • Demonstrates respect for and promotes dignity of patient in all interactions.
  • Maintains confidentiality of patient information as appropriate.


QUALIFICATIONS:


  • Current Oregon licensure as a Registered Nurse. 
  • Experience as a staff nurse in an acute care setting or Home Health Agency or Hospice.
  • Have availability of an automobile and liability insurance; and have current Oregon driver’s license.
  • Satisfactory results from a background check and drug screen required.

  • Ability to function effectively in high stress and crisis situations. Ability to manage multiple priorities, and work as a team participant.
  • Ability to assess and respond to the needs of patient and their families in any setting where the patient might be located.
  • Demonstrates self-management skills related to personal loss issues in order to enable self to be an effective participant in the Hospice setting.  
  • Demonstrates computer literacy in order to accurately document patient visits as necessary.
