JOB TITLE: Social Worker – LCSW / MSW

RESPONSIBLE TO: Patient Care Manager and Social Worker (LCSW)

POSITION SUMMARY: Responsible for providing high-quality social work services to hospice patients and their families, including limited bereavement services.

ESSENTIAL JOB FUNCTIONS:

1. Conduct comprehensive social work assessments of patient/family units to determine needs, dynamics, and coping strategies.
2. Accurately document findings promptly, ensuring compliance with hospice standards and reflecting factual interactions with patients and families.
3. Provide bereavement support services as needed, including initial bereavement assessments to evaluate patient/family social and emotional factors, assessing their ability to cope with terminal illness and death.
4. Conduct regular clinical social work supervision and training for non-licensed social work team members.
5. Regularly provide clinical social work supervision and feedback to the Bereavement Coordinator, particularly in cases of complicated grief among bereaved individuals.
6. Foster continuity of patient care and therapeutic intervention by developing care plans and communicating essential information to patients, families, team members, and attending physicians as needed.
7. Coordinate the utilization of community resources to address the social, emotional, physical, and financial needs of patients and families.
8. Provide psychosocial counseling and support to patients and families navigating emotional, social, and economic challenges.
9. Participate in the admission process, including planning and coordinating visit schedules, considering patient needs, acuity, and location.
10. Educate patients and families on hospice benefits, explaining details and obtaining necessary consents. Collaborate with the Interdisciplinary Team (IDT) to coordinate staff and community services based on patient/family needs.
11. Engage in interdisciplinary team (IDT) meetings to establish, review, and revise patient care plans, exchanging information, addressing concerns, and providing professional support.

12. Follow up and revise care plans in response to evolving patient and family needs, ensuring individualized and timely adjustments under the guidance of the IDT
13. Advocate for patients’ rights, assisting them with Advanced Directives and POLST forms as requested, respecting their autonomy.
14. Demonstrate dedication to enhancing service quality and participate in the Quality Assessment Performance Improvement Program (QAPI) as required.
15. Integrate QAPI principles into social work and bereavement care components.
16. Exhibit flexibility and undertake additional duties as assigned.
17. Attend mandatory in-service education sessions.
18. Maintain confidentiality of patient information in compliance with regulations.

Job Expectations:
1. Uphold patient and family confidentiality in accordance with HIPAA regulations.
2. Demonstrate strong self-management skills, including time management, communication, and prioritization of tasks in a multitasking environment.
3. Successfully collaborate with various organizations and individuals.
4. Possess effective organizational, assessment, and communication skills.

Qualifications:
1. Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on
Social Work Education or

  • Has a baccalaureate degree in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or
  • A baccalaureate degree in psychology, sociology, or other fields related to social work and has 1 year of social work experience in a healthcare setting.

2. Minimum one year of experience in a healthcare setting; hospice or home health experience preferred.
3. Familiarity with community resources, including Medicare/Medicaid, financial aid, and county, state, and private social services agencies.
4. Ability to work independently, with demonstrated skills in patient care planning, patient education, and strong written and verbal communication abilities.
5. Must have general computer skills and a working knowledge of MS Office, Outlook, and Electronic Medical Records
6. Valid Driver’s License with a reliable and insured vehicle.

Pay: $31.00 - $51.00 per hour
