Hospice of San Joaquin is the oldest not-for-profit hospice of 40 years. We serve all of San Joaquin and Stanislaus Counties. We are seeking a Dynamic, self-motivated CNA who thrives in a fast-paced environment and to support our mission. This is a Part-Time - PM Shift position reporting to the Director of The Hospice House.
POSITION PURPOSE/SUMMARY:
As a member of the multi-disciplinary team, the caregiver/CNA will provide personal care services to the terminally ill patient/family in accordance with a written plan of care.
AREAS OF RESPONSIBILITY/ACCOUNTABILITY CLINICAL SERVICES:
Responsible to the LVN on duty, and/or the Hospice House Administrator for carrying out the duties assigned in order to increase the level of comfort, and to maintain personal hygiene and a safe, healthy environment for the patient.
1. Services are carried out according to Hospice of San Joaquin's policies.
2. Provide or assist patient in bathing, care of mouth, skin, or hair.
3. Assist patients in and out of bed and assist with ambulation.
4. Turn patients in bed and positions for comfort.
5. Assist with prescribed exercises which patients and Health Aide Caregiver/CNA have been taught by appropriate health personnel.
6. Prepare and serve meals and assist patients with eating.
7. Assist patients to the bathroom or in using bedpan or urinal.
8. Give enemas as ordered and instructed.
9. Perform household services which will facilitate the patient’s self-care at home as necessary to prevent or postpone institutionalization.
10. Change bed linens.
11. Take and record vital signs as directed.
12. Complete appropriate records.
13. Adheres to all Hospice of San Joaquin policies, and procedures, including following Hospice of San Joaquin infection control procedures and the code of ethics.
14. Other assignments may be appropriate.

Qualifications
JOB QUALIFICATIONS
1. Good verbal and written communication skills are required.
2. Demonstrated ability to make meaningful observations and write a brief, accurate reports.
3. Demonstrated ability to recognize the needs of people and to maintain good relationships in dealing with them.
4. Experience as health aide/CNA with active CNA/Home Health Aide license
5. Ability to deal effectively with stress.
6. High school graduate or equivalent degree.
7. Flexibility.
OTHER QUALIFICATIONS
1. Good personal health and physical and emotional stability.
2. Physically able to perform and assist with resident’s personal needs and care of residence including, standing, lifting, reaching, bending, stooping and carrying.
3. Ability to safely maneuver equipment (wheelchairs, etc.)
