The Medical Records Clerk is responsible for coordinating hospice patients' medical records and office functions in accordance with state, federal, and local regulations. This role requires a high school diploma, with college education preferred. Applicants must have a valid Texas Driver's License and reliable transportation with valid auto liability insurance. While two years of general office and medical records management experience is preferred, the position requires strong computer skills, excellent interpersonal and organizational abilities, and keen attention to detail. Knowledge of office machines is beneficial.
The role involves prolonged periods of sitting, with some standing, and occasional lifting, pulling, carrying, and pushing items up to fifty pounds. The Medical Records Clerk will frequently stoop, kneel, and reach to access files. Hand-eye coordination and manual dexterity are essential for handling records efficiently. Working under some stressful conditions to meet deadlines is expected. The work environment is a routine office setting, occasionally involving moderate noise levels, flexible scheduling, extended hours, local travel, and occasional exposure to inclement weather.
The Medical Records Clerk's responsibilities include overseeing agency communications such as electronic medical records (EMR), telephones, mailing, and tracking physician orders. They ensure compliance with state and federal regulations and maintain up-to-date knowledge of hospice regulations. Duties also include maintaining medical records per regulatory standards, managing information releases with signed authorizations, gathering patient demographics, and setting up EMRs promptly. The role supports audits, participates in the quality assurance program, protects medical records confidentiality, and assists with clerical tasks such as answering phones, responding to communications, and processing patient admission and discharge records.
