Hospice Field Staff RN/ Case Manager Position Summary and Details
- Position Summary: The Staff Registered Nurse is responsible for assessing the patient/family/caregiver needs, planning for the care, implementing the Plan of Care, and evaluating the effectiveness of the care provided. As a member of the IDT, the Staff Registered Nurse/Case Manager oversees the direction and coordination of a patient’s care throughout their time in hospice.
- Pay: $38.20 Hourly
- Full-Time Non-Exempt Position, 40 Hours Weekly
- Schedule: Monday- Friday 8:30 AM- 5:00 PM with an additional rotational on-call schedule that includes some evening, weekends, and holidays.
- Will be based out of our Belleville Patient Services Office.
- Would travel within the Belleville office’s service area western service territory which include the following counties: St. Clair, Monroe, Randolph, Washington, Clinton
- We visit our patients wherever they may located, whether it be in their homes, long-term care facilities, nursing homes, hospitals, etc. As a result, a valid driver's license, current automobile insurance, and reliable transportation are requirements of the position. The company reimburses .67 per mile for driving personal vehicles.
