JOB SUMMARY:
The GIP Referral/Admissions RN maintains organization of new GIP referrals. Including accepting new referrals, data entry of new referrals, filing/scanning paperwork in the charts, and coordinating communication between referral sources and Hospice MD, billing.
He/she will have an impact on Referral sources, potential patients and families and Hospice staff. This person will need a courteous, organized and patient manner along with a working knowledge of hospice and qualifying criteria for GIP status.
RESPONSIBILITY (not limited to)
· To receive referral phone calls/faxes and speak with families, appropriately
· To assist with computer data input, i.e. new referrals
· To communicate with referral source Social worker, Nurses and Case Managers
· To be knowledgeable in terminal disease processes and be able to read and decipher H&Ps to determine eligibility
· To communicate with Hospice MD to discuss case, accept or deny referral and input orders/notes as needed
· To complete In-person patient assessments at local facilities to determine qualification for GIP admission to the Hospice House or Inpatient at local hospital
· To complete assessments via telephone with families and nursing staff of out-of-town facilities to determine qualification for GIP admission to Hospice House
· To confirm insurance and billing information and be knowledge about it’s implications to patient situation
· To provide EOB to family and complete admission documents on acceptance
· To maintain flow of information to families during transition from acute care to Hospice services/IPU
· To ensure timely communicate with Hospice House on bed status/availability and to give report on accepted referrals
· To maintain communication with Intake Coordinator on new referral, acceptance or declination of new referral
· To assist with uploading paperwork in patient charts and ensuring that all required components of chart are filed timely.
· To make daily rounds and enter notes on GIP patients admitted to local facilities and communicate daily with their families
· To work closely with nursing supervisors
· During periods of low referrals, RN will assist with Hospice House patient care or Home Team routine visits as directed by Directors of Clinical Services

PHYSICAL DEMANDS and WORKING CONDITIONS
Requires sitting for long periods of time; occasional bending, stretching or lifting. This position requires the ability to work under high stress conditions for long periods of time. Ability to hear normal range of voice, drive, and negotiate steps, walkways or housing not designed for handicap access. May require contact with individuals with communicable disease. Able to work in normal office environment, hospitals, homes, use office equipment, and prepare written reports.
