What are the responsibilities and job description for the Admissions/Transitional Care Nurse position at Hospice Of Southern Maine?
Hospice of Southern Maine is seeking a full time Admissions/Transitional Care Nurse to work in our Access Department. (Monday through Friday10am-6:30pm and one weekend day every 4 weeks).
As a key member of the Access Department RN team, the Admissions/Transitional Care Nurse (ATCN) is responsible for assuring that Hospice of Southern Maine’s programs are effectively represented and accessible to patients, families, referring physicians, and others with an interest in hospice through pre-admission evaluations, informational visits, intake and referral coordination, admissions, other RN assessment visits as needed, and community outreach and education. The ATCN will also help facilitate transitions of care for hospice-eligible patients in hospitals. The ATCN works closely with HSM’s Access Department, Medical Directors, and referring physicians to ensure timely and responsive access to hospice care according to agency mission and goals.
Graduate of an NLN accredited nursing program; BSN or CHPN preferred. Minimum of two years of RN experience required. Intake/access, home care, hospice, hospital and/or oncology nursing experience preferred. Registered Nurse licensure by the State of Maine; valid driver’s license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order. BLS certification or the ability to obtain certification within the first 90 days of employment required. Certification must be kept active during the course of employment.
