What are the responsibilities and job description for the Hospice RN Team Manager position at Hospice of Southwest Ohio?
Hospice of Southwest Ohio is growing! We are looking for a dynamic RN to join our clinical management team. This RN Team Manager will coordinate and oversee direct and indirect patient services as for ensuring that care and services are delivered. Our Managers enjoy a Monday - Friday schedule with a minimum weekend and holiday requirement. Position requires 2 years of hospice experience in a hospice with management or supervisory experience in hospice preferred.
1. Provides help in assessment, planning, implementation and evaluation of patients. Leads weekly IDT meetings in accordance with CMS regulations. Ensures team members are in compliance with CMS regulations by completing on going Quality Assurance and Performance Improvement audits.
2. Assist the Director of Clinical Services in the preparation and the administration of the budget for the hospice operation.
3. Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses.
4. Assists in interviewing and hiring hospice personnel. Provides education to employees and assists in orientation and ongoing education. Maintains records pertaining to orientation and education for assigned team members. Competes evaluations of personnel as directed by Human Resources Director.
5. Assists the Director of Clinical Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in the formulation, revision, implementation, and evaluation of organizational policies, procedures, goals, objectives, both short and long range.
6. Conducts annual evaluations of personnel, or more frequently if indicated.
7. Oversees the maintenance of patient clinical records, statistics, reports, and records for purposes of evaluation and team activities.
8. Assures the proper maintenance of patient clinical records in compliance with local, state and federal laws.
9. Responsible for the maintenance of an adequate and appropriate inventory of supplies and equipment for the provision of patient care services.
10. Complies with the organizational standards of professional conduct.
11. Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for hospice nursing.
12. Serves as a clinical resource for nursing personnel.
13. Assumes patient care assignment as patient census and staffing levels dictate.
14. Assesses the health needs of the community and assists in the development of plan (s) or programs (s) to meet those needs. Participates in public relations and community activities that promote the organization’s role as an effective member of the hospice and health care delivery system. Promotes customer service orientation to all Care Center personnel.
15. Assumes responsibility for personnel growth and development maintaining and improving professionals knowledge and practice skills through attendance and participation in continuing education programs.
16. Performs other job related tasks as assigned by the Director of Clinical services.
17. Reviews available patient information related to all new admissions to determine hospice needs.
18. Reviews and evaluates the hospice diagnosis, prognosis, medication record & procedures for clinical course of each patient residing in hospice care for appropriateness.
19. Performs monthly patient chart audits for all disciplines. Reports findings of the audit with recommendations to the Chief Nursing Officer in addition to the hospice personnel. Participates in monthly QAPI meetings to discuss data.
