What are the responsibilities and job description for the Nurse Practitioner, Palliative Care position at Hospice of Southwest Ohio?
CareBridge Palliative Services is a locally owned and operated company in search of a part-time community based Palliative Care Nurse Practitioner for Butler County and Western Hamilton County.
You must be highly motivated, have a keen knowledge of chronic illnesses, along with medications, you must be comfortable with goals of care conversations. Palliative services are for any person suffering from a serious or chronic illness with life-limiting symptoms. At Carebridge, we consider these to be any physical, emotional, or psychosocial symptoms impacting quality of life. The Nurse Practitioner will maintain a community-based case load.
The Nurse Practitioner is part of the Carebridge Palliative Care and Hospice of Southwest Ohio interdisciplinary teams and receives back-office support for patient scheduling, billing, and other administrative needs.
Schedule follows a full time Monday-Friday work schedule with on call duties.
Job Duties Include:
· Review goals of care and provide education on disease progression.
· Identify and monitor interventions to limit symptoms resulting from the palliative diagnosis.
· Coordinate with other members of the interdisciplinary team as needed for admission to Hospice and/or discharge from Palliative Care to other level of service in the community setting.
· Completing documentation in the electronic medical record system in accordance with state and federal regulations; submitting documentation for claims billing timely each month.
· Educate patients and their family members on diseases, and prognosis
· Active DEA License required.
· Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth.
Experience in Hospice, Palliative Care, chronic illness management, or relative medical setting is required.

Minimum Education: Graduate of an accredited school of nursing; registered nurse practitioner with a current State of Ohio license.
Minimum Experience: One year experience in a home care, hospital, hospice or palliative care setting.
We offer an excellent benefit package including PTO accrual starting from date of hire. Benefits are eligible after 30 days from date of hire following the first of the month.
