Hospice Case Manager (RN)
Hospice of Southwest Ohio is growing! We are a locally owned and operated hospice that is deeply rooted in our community. We are looking for a compassionate, experienced RN Hospice Case Manager to join our team. Our RN Case Managers take no after hours call and enjoy a Monday thru Friday schedule. We value a work/life balance.

The registered nurse plans, organizes and directs the care of the hospice patient and is experienced in nursing with an emphasis on community health education/experience. This professional nurse builds from the resources of the community to plan and direct these services to meet the needs of the patient with them individually and with their families focused within the patient’s home and their community.
