What You'll Do :
- Support & Advocate - Assess patients and families, develop personalized care plans, and provide emotional and practical support to enhance coping skills.
- Educate & Guide - Help families navigate the dying process, anticipatory grief, and bereavement while ensuring they have the resources they need.
- Collaborate & Communicate - Work closely with an interdisciplinary team, including nurses, chaplains, and volunteers, to advocate for patient needs.
- Connect to Resources - Make referrals to community organizations, chaplains, and volunteer programs to provide additional support.
- Stay Organized - Maintain timely and accurate documentation to ensure seamless care for each patient.
