The Nurse Practitioner for Golden Isles Palliative Care under the direction of the Chief Medical Officer has overall responsibility for the palliative care direction of patients and their families in collaboration with, the Nurse Navigator, according to the patient’s plan of care and with other providers in which the patient may be engaged.
S/he functions as a consultative resource for Palliative Medicine performing skilled clinical assessments and formulating treatment plans. Assists in the growth and development of the new Palliative Care initiative expanding community reach and effectiveness over time.
Position Details
- Work Location: Southeast Georgia Health System Outpatient Clinic (2500 Starling St, Brunswick)
- Days/Hours: Monday - Friday, 8:00am-5pm usually
- Compensation: $95,000 - 105,000; full benefits package including 23 PTO days annual
Job Responsibilities
- Ensures proper evaluation of patients to determine eligibility for Palliative Care services.
- Develops and initiates comprehensive plans of care for patients. Assesses acute and non-acute clinical problems; documents physical assessments and patient histories, analyzes trends in patient conditions, develops and documents patient care plans including in-depth progress notes and dictation summaries.
- Provides guidance on nursing plan in addition to patient/family counseling and education.
- May write admission, transfer, discharge orders, order and interpret laboratory and diagnostic studies, may order appropriate medications and treatments, may refer patients for consultations for hospice services.
- Promotes access to palliative care and provides education to medical professionals and referral sources to develop and maintain the Golden Isles Palliative Care organization.
- Performs regularly scheduled hospice rounds of inpatient and residential patients under the supervision of the CMO. Conducts hospice home visits as needed. Documents according to policies and procedures.
- Covers hospice inpatient services /call as needed including weekend coverage under the supervision of a physician.
- Assists with hospice communications, medical record review, and orders under the supervision of hospice physician.
- Educates community and local physicians about hospice and palliative care. Develops a collaborative palliative care program with the local hospital system.
Education & Certifications
- Licensed as current APRN/FNP license in the state of Georgia.
- Completion of an accredited Nurse Practitioner degree program.
- Clinical experience and credentialing in Hospice and Palliative Care is preferred.
- Advanced nursing knowledge with several years of nursing and medical practice and at least 1 year as a Nurse Practitioner.
- Updated BLS certification.
- Member in good standing on the medical staff of the local accredited hospital.
Desired Knowledge & Skills
- Knowledge of a wide range of nursing and medical principles, theories, techniques and practices required to perform clinical evaluation and medical management of patients with palliative care needs.
- Knowledge of clinical research in addition to the biological aspects of the disease process.
- Ability to work successfully within an interdisciplinary team to achieve positive outcomes for patients and their families.
- Ability to represent HGI to the general public and within the physician community.
