Description
Hospice of the Piedmont is seeking a Admissions Nurse for a PRN position to serve our community with world class hospice care.
Responsibilities/General Duties:
- Maintains ongoing public relations, hospice advocacy and communication with physicians, facility staff, and other referral sources, regarding Hospice of the Piedmont services
- Visit every referral as assigned by Admission Intake Coordinator at set time or call if arrival is delayed and be responsible for that patient through to admission or referral back to the community
- Provide initial hospice presentation to patient and their circle of support, assess their understanding of the hospice benefit, and actively advocate for hospice admission of medically eligible patients
- Gather information as needed to include but not limited to demographics, insurance, clinical history, medications
- Perform nursing admission assessment visit with medical eligibility based on agency criteria, assuring compliance with all Federal, State and Hospice regulations/policies
- Initiates the plan of care in conjunction with physician orders, patient/family needs, and in coordination with other interdisciplinary team members
- Practices appropriate and timely documentation at the bedside or immediately following the visit in preparing clinical notes which clearly identifies teaching, communication, and plan for Hospice services
- Acts as resource to community, physicians, hospital or nursing home staff in educating them as to Hospice mission and philosophy, Medicare/Medicaid benefit and any of HOP’s programs of care
- Encourages building referral base through actively visiting potential new sources providing information and education
- Attends required meetings, conferences and workshops and participates in committees as requested
- Participates in professional development activities and maintains professional affiliations
- Adheres to the practice of confidentiality regarding patients, families, staff and the agency according to HIPAA regulations
Qualifications:
- Bachelor or Associate degree in Nursing or equivalent from an accredited school of nursing
- One year experience in Hospice or Home Heath required
- Demonstrates proficiency with EMR software, basic to intermediate proficiency in Microsoft Outlook, Word and Excel. Must demonstrate basic computer skills.
- Licensed as a Registered Nurse in the Commonwealth of Virginia and CPR certification current
Typical Physical Demands/Working Conditions:
- Sit, stand, stoop, bend, reach, lift and walk, climb steps
- Communicate by phone, writing, speaking
- Lifting up to 20 pounds
- Drive short/long distances
- General fast-paced work environment with occasional stressful situations
- Must be prepared to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens smoke, no chairs
- Must be prepared to complete EMR at bedside in varied environments
- Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
Other:
- Requires valid VA driver’s license, personal automobile and current automobile liability insurance
- Frequent travel may be required
