What are the responsibilities and job description for the Hospital Liaison (FT) position at Hospice of the Piedmont Inc?
Hospice of the Piedmont is seeking a full-time Hospice Nurse Liaison covering Culpeper Medical Center and Fauquiar Hospital to coordinate all services related to individuals that have inquired about or been referred to hospice services. Provide the initial hospice presentation to patients and their circle of support, assess their understanding of the hospice benefit, actively advocate for hospice admission of medically eligible patients. Advocate for and promote the gift of hospice throughout the extended care community through legendary customer service and education of families, patients and hospital staff.
Responsiblities/General Duties (Including but not limited to):
- For the purpose of improving care of the terminally ill, develops a hospice presence in the hospital, attending rounds and care conferences when possible, conferring with providers, speaking at in-service meetings, attending committee meetings as assigned and participating in new staff orientation.
- Working in conjunction with hospital case managers, coordinates timely discharge plans with patients/families, hospital and hospice staff for newly referred patients. Provides consultation and follow-up as requested. Assesses each consult to determine the most appropriate disposition plan is executed which serves both patient and the health system.
- Informs appropriate hospital patients and their families about Hospice of the Piedmont services.
- Develops, educates, implements, and serves as a consultant to the hospital medical professionals concerning Hospice of the Piedmont services, pain management and the hospice as well as Medicare/Medicaid/commercial reimbursements. Advises the Hospital Relations Manager and of medical community needs and makes suggestions regarding meeting those needs.
- Provides input into marketing and business development plans.
- Responsible for census growth.
- Demonstrates a strong familiarity with policies and procedures of agency, state and federal regulations regarding hospice.
- Transfers patient information from hospital system to hospice EMR in a timely manner.
- Acts as a liaison and advocate for current hospice patients who are admitted to the hospital or in the ER.
- Participates and assists in any research, QI, or other studies involving end of life care as requested.
- Adheres to the practice of confidentiality regarding patients, families, staff, and the Agency.
Qualifications:
- Graduate of an accredited nursing school; licensed as a Registered Nurse in the Commonwealth of Virginia.
- Two or more years of relevant nursing experience in terminal care, health team service coordination, documentation and case management. Previous experience in hospice, home health, health care sales or marketing is desirable.
- Preferred strong knowledge of hospice regulations and reimbursement issues surrounding hospice care in the nursing home. Hospice of Piedmont will train if there are knowledge gaps.
- Ability to manage all aspects of terminal care, health team service coordination, and documentation. Knowledgeable of organizational policies, procedures, systems and objectives.
- Emotionally mature and caring individual willing to give support and assistance to terminally ill and their families.
- Basic to intermediate proficiency in word processing software. Must demonstrate basic computer skills.

- General fast-paced work environment with occasional stressful situations
- Must be prepared to complete EMR at bedside in varied environments
