Hospice of the Piedmont Inc is seeking a full-time RN Case Manager to manage cases and provide quality patient care to our hospice patients residing in long-term care and assisted living facilities.
**Key Tasks:**
- Perform nursing evaluation visits to new admissions.
- Determine eligibility based on agency criteria.
- Collaborate with the interdisciplinary team to develop the Plan of Care and Nursing Care Plans.
**Requirements:**
- Graduate of an accredited nursing school.
- Licensed as a Registered Nurse in the Commonwealth of Virginia.
- Two years of relevant nursing experience.
