Schedule : Full-time, hourly, non-exempt role. Monday-Friday, 8am-5pm
This Referral Specialist is responsible for supporting the Referrals and Admissions Department through scheduling, coordinating, and managing the referral intake process. This position is responsible for assisting the department in admitting patients onto agency services including hospice, palliative care, Staywell and other programs offered by Hospice of the Piedmont.
Here are the essential duties day to day :
- Assists with referral intake process by providing program information to prospective patients, families, and referral sources. Provides information, collects information, and develops the referral according to agency procedure.
- Accepts referrals through various methods of communication including telephonic inquiries, fax transmissions, walk-ins, email, and other electronic submissions.
- Obtains and gathers relevant medical information related to the patient. Requests medical records from physicians, hospitals, long-term care facilities, and other healthcare entities to support the Referrals and Admissions team in determining eligibility of the patient.
- Inputs information into our electronic medical record including a referral note about the patient. Other required information includes but is not limited to : including basic demographic information, insurance and payer sources, admission / status referral codes, and orders to admit from MD.
- Schedules the admission for the Nurse Liaison, Care Coordinator, or other designee and assists in arranging services for the patient. Position is responsible for communicating with the patient, family, primary care physician, and other involved parties to ensure an easy transition.
- Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
- Assists with audits as assigned.
