Duties include but are not limited to:
- Provide a nursing evaluation visit to new admissions
- Regularly re-evaluate patient needs
- Consult with the patient’s physician
- Complete bedside documentation using the EMR
- Coordinate and manage services offered by health team members
- Help ensure the final chapter of your patient’s life is one of quality, compassion and dignity
Requirements
The RN Case Manager position requires graduation from an accredited nursing school and a valid license as a Registered Nurse in the Commonwealth of Virginia; two years relevant nursing experience preferred.
Typical Physical Demands/Working Conditions:
- The physical demands may require employee to regularly use hands to finger, handle or feel objects, tools, or controls, reach with hands and arms; Sit, stand, stoop, bend, reach, lift and walk, climb steps. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth vision, and ability to adjust focus
- Communication by phone, writing, speaking; Regular use of computer screen and keyboarding are required
- Lifting to 20 pounds, push up to 200 pounds in a wheelchair
- Drive short/long distances
- General fast-paced work environment
- Physical and emotional health necessary for the accomplishment of duties

- Driver’s license, personal automobile, and current automobile liability insurance
- Frequent travel may be required
