We are currently hiring a Full-Time RN Case Manager to provide Hospice Care!
What You’ll Do:
- Lead patient care planning, ensuring comprehensive and individualized care.
- Assess, monitor, and manage patient health, making adjustments to care plans as needed.
- Collaborate with a multidisciplinary team to provide holistic and compassionate care.
- Support patients and families with education on end-of-life care, managing symptoms, and emotional well-being.
- Be a comforting presence, offering empathy and guidance through difficult times.
- Maintain compliance with regulatory standards and best practices in hospice care.

Qualifications:
- Must be a Registered Nurse (RN) in the State of Colorado
- Must maintain a valid driver's license in the State of Colorado and automobile insurance
- Previous experience in hospice nursing or other high acuity medical fields preferred but not mandatory
- Familiarity with electronic medical records and equipment a plus
- Ability to build rapport with patients and families
- Strong problem-solving skills
- Ability to work independently
- Ability to thrive in a fast-paced environment
