Position Summary
- Responsible for providing excellent medical care to primary care patients
- Collaborates with multi-disciplinary teams
- Requires a high level of communication that is clear, concise, patient-centered; team education/guidance is vital to our patient care and mission accomplishment
- Significant travel is required; vehicle will be provided or mileage reimbursed
- Care will be delivered in compliance with all pertinent regulations including the hospice conditions of participation, corporate compliance, quality standards, licensure regulations and will reflect the national standards of care for hospice services
This position is full-time, benefit eligible, and will be based out of our Bismarck office.

Qualifications
Basic Qualifications
- Nursing Practitioner training from accredited institution of higher learning
- Three years of progressive clinical/medical practice based on current research, trends, and best practices
- Solid understanding of palliative and hospice care as it relates to community healthcare organizations
- Solid understanding of palliative and hospice needs from profit, non-profit, federal healthcare sectors, including needs of patients and family/caregivers wherever the patient calls home (Including assisted living, and varieties of nursing facilities)
Certifications, Licenses, Registrations Required
- DEA
- Licensure in North Dakota and Minnesota with ability obtain licensure in other states as required
- Current drivers license and auto insurance
