Seeking a reliable, hardworking, team oriented Personal Care Aide for our Fargo region. As a Personal Care Aide this person will be responsible for providing personal care, home management, and essential transportation services. These services could include personal hygiene care, delegated medical care, housekeeping, laundry, shopping, essential non-medical transportation, and more. This position will also assist as a hospice nursing assistant being a vital member of the interdisciplinary (IDG) team, providing personal care services, obtaining vital signs, and completing charting timely for hospice patients.
This person must work within the nursing assistant job scope to meet the requirements of applicable federal, state and local regulatory agencies.
KEY RESPONSIBILITIES – Personal Care Aide
- Providing personal care such as - bathing, dressing, toileting, incontinence management, transferring/ambulating, eating, personal hygiene, etc.
- Completing home management tasks - housekeeping, laundry, and shopping
- Transporting to essential community services such as grocery stores, pharmacies, banks, post offices, etc
- Escorting the individual to assist with completing their task once arrived at destination
- Performing delegated medical care tasks such as dressing changes, compression wraps/stockings, catheter cares, nail care, etc.
- Completing Electronic Visit Verification and appropriate documentation for all services provided
- Inspects vehicle to ensure that it is in a clean and in operable condition and reports the need for any service or repair
- Routes daily trips in an efficient, effective and safe manner.
- Maintains vehicle logs and checklists as required
- Communicates and reports any client welfare concerns or unusual circumstances to Supervisor.
- Washes and cleans the interior of the vehicle; fuels the vehicle.
- Participates in in-service training to update skills and learn techniques for dealing with various clients.
- Obtain vital signs and weight and documents or reports to nursing staff.
- Works effectively as a team member in the delivery of care and assists in the orientation of new employees. Maintains good working relations with facility employees and patient family members through appropriate communication.
- Instills confidence in patients by maintaining a neat appearance and performing duties in a competent and caring manner.

Qualifications
REQUIRED EDUCATION, CERTIFICATIONS, AND EXPERIENCE
- Certified Nursing Assistants registration from North Dakota and Minnesota State Department of Health - preferred
- Valid drivers license and auto insurance - required
PREFERRED QUALIFICATOINS
- Must be 25 years of age or older
- Two years’ experience driving and transporting patients
- One year patient care experience in a health care setting; hospice or home health experience preferred
- High School Diploma/GED
- Ability to be flexible and effectively manage priorities, multiple tasks and meet deadlines
- Ability to work with and communicate effectively with terminally ill patients and their families
