ADMISSIONS NURSE
REPORTS TO:
Patient Access Manager
JOB GOAL: To perform a comprehensive nursing assessment and establish the initial plan of care, education, and interventions to provide for patient and family directed care, comfort, and quality of life. Assist in determining hospice eligibility.
MINIMUM QUALIFICATIONS:
1. Is a graduate of an accredited school of professional registered nursing, BSN preferred; but not required
2. Is currently licensed as a Registered Nurse through the State Board of Nursing; and
3. Have had at least one (1) year of Hospice, Home Health, or general Medical-Surgical experience in a clinical care setting including at least one (1) year of Admissions experience; and
4. Possess and maintain a current CPR Certification; and
5. Must have and maintain a current Alabama driver’s license; and
6. Must have and maintain current liability insurance on vehicle; and
7. Must successfully complete the post job offer physical and submit to periodic tuberculin skin tests or chest x-rays at agency expense.
8. Must possess and maintain good physical stamina and mental health.

ADDITIONAL DESIRABLE QUALIFICATIONS:
1. Possess an understanding and is committed to the Hospice philosophy and all
aspects of issues related to end-of-life care.
2. Has excellent assessment, good nursing judgment, and communication skills.
3. Is self-directed with the ability to work with little supervision.
4. Experienced with utilizing electronic medical records (EMR) systems to store,
access, and update patients’ health records electronically.
ESSENTIAL FUNCTIONS

1. Conducts admissions ensuring patients and families being referred to Hospice of the Valley understand services to be provided and qualifies under guidelines.
2. Provides consultative, informational visits as ordered by physician to patients/families who are interested in Hospice services.
3. Serves as a liaison between the hospital/ALF/SNF/LTC facilities and Hospice of the Valley regarding referral of patients who need direct admission or who are in need of home care services upon discharge from the facility.
4. Must be flexible to admit patients after hours and on weekends if needed.
5. Performs comprehensive physical assessment and initiates appropriate symptom management intervention during admission process.
6. Coordinates all appropriate medication, medical equipment and supply acquisition as needed, independent of, or in coordination with Admissions Social Worker and/or Clinical Team Assistant.

7. Following admissions, completes and submits admission documentation within appropriate time frame.
8. Following admission, gives pertinent, concise admission follow-up report to designated Hospice of the Valley staff and appropriate referral source.
9. Participates, when possible, in IDG – reporting on recent admissions.
10. Communicates outcome of admission with Patient Access Manager and assists with follow-up on non-admits.
11. Evaluates the referral/admission process and makes recommendations for change when appropriate.
12. Assist Patient Access Manager with intake and coordinating evaluations, consultations, and admits. Includes contacting P.C.P., families, and other providers.
13. Shares coverage of back-up weekend and holiday on-call service.
14. Assist Patient Care Services with assessment and care of home care patients as appropriate.
15. Participates in education/ in-service programs for staff as directed.
16. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
17. Understands that teamwork is a crucial part of our business, and can work well with colleagues, no matter the circumstances. Using individual skills, cooperates with others, and accepts and provides constructive feedback despite any personal conflicts between individuals involved.
18. Utilizes current Agency and/or department specific software to complete assignments.
