Job Summary
We are seeking a Patient Care Specialist to join our team. The ideal candidate will be responsible for providing personal care under the supervision of a nurse, collaborating with a transdisciplinary team, and promoting quality of life for patients.
Duties and Responsibilities
- Provide personal care and assist with Activities of Daily Living (ADLs) according to the patient's plan of care.
- Collaborate with the healthcare team to provide safe and effective care.
- Assist with passive and active Range of Motion (ROM) exercises as directed by the nurse.
- Prepare nutritional snacks for patients as needed.
- Record observations and activities provided in the patient's clinical record.
Requirements
- State-tested nursing assistant in good standing.
- Ability to read and write English proficiently.
- High level of integrity, ethics, and professionalism.
