Job Responsibilities
- Assume primary responsibility for patient caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process.
- Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient's needs and initiate intervention by other members of the interdisciplinary team.
- Collaborate with healthcare providers to develop and implement individualized care plans that meet the unique needs of each patient.
Requirements
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Current CPR and First Aid Certification
Salary Information
$65,000 - $80,000 per year, depending on experience
Hospice Certification Benefit and Salary Increase Available
