About the Role:
- This position requires a graduate from an accredited registered nursing program and a valid driver's license with proof of insurance.
- You will be responsible for initiating communication with the attending physician, interdisciplinary team, and other agencies for coordinating optimal care.
- Maintain accurate patient records, charts, progress notes, reports, and prepare care plans timely.
Job Responsibilities:
- Provide regular visits to re-assess the patient's needs and update the plan of care.
- Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines.
