Schedule: Full-Time | 2:00 PM – 12:00 Midnight (2-week rotating schedule)
Holiday Commitment: One major and 2–3 minor holidays annually
Location: Patient homes and facilities throughout our service area
Job Summary
This full-time professional RN position is responsible for completing the admission, initial assessment, and related documentation for new hospice patients. This role requires a blend of clinical expertise, compassionate care, and strong coordination skills, all carried out in alignment with Transitions LifeCare’s mission, values, and regulatory standards.
Responsibilities
Conduct initial clinical assessments and develop care plans for elective, urgent, and emergent admissions.
Collaborate with physicians, families, and clinical teams to coordinate care, including alternative arrangements when admissions are delayed.
Accurately gather and document insurance information; refer self-pay cases to financial screening.
Provide and document safe, compassionate, and compliant hospice nursing care.
Deliver patient and family education regarding end-of-life care.
Contribute to performance improvement initiatives and mentor new staff.
Manage timely documentation and safe use of medications, supplies, and equipment.
Work collaboratively with the interdisciplinary group (IDG) to support high-quality patient care.
Requirements
Current RN license in North Carolina (BSN preferred)
At least 2 years of recent clinical experience (hospice, home health, or nursing home strongly preferred)
CHPN certification preferred
Expertise in clinical assessment, medical terminology, and end-of-life care practices
Strong computer skills (Netsmart, Office 365, Outlook preferred)
Current CPR certification and valid driver's license with reliable transportation
Must be able to travel to patient homes and facilities anywhere in our service area
Must be able to lift/carry up to 50 pounds and work in diverse home/facility settings
Environmental Considerations
Exposure to bloodborne pathogens and communicable diseases may occur
Must have sensory abilities for physical assessments and documentation
