Part-Time Nurse Practitioner - Palliative Care
Job Summary
Transitions LifeCare is seeking a Part-Time Nurse Practitioner to provide Palliative Care Consultation in the hospital setting. This role involves providing direct patient care, working with the Director of Inpatient Palliative Medicine, and educating hospital staff and community physicians about pain and symptom management, end-of-life communication, and available Transitions LifeCare programs. Schedule : Saturdays and Sundays, 8 : 30 am - 5 : 00 pm, with holidays as scheduled.
Key Responsibilities
- Provide palliative care consultations to patients in the hospital setting.
- Deliver ongoing direct patient care under the clinical supervision of the Director of Inpatient Palliative Medicine, utilizing evidence-based interventions for pain and symptom management.
- Educate hospital staff and community physicians on palliative care, pain management, and end-of-life decision-making.
- Attend IDG meetings and document care in patient medical records to meet regulatory and agency guidelines.
- Assist with data collection and outcome evaluations for the Palliative Care Program.
- Participate in on-call rotation and other duties as assigned.
