About the Role
Join our Transitions Kids interdisciplinary team as a Pediatric Palliative Care Specialist (RN) , providing compassionate, patient-centered care for children with serious illnesses. You’ll collaborate with families and care teams to develop and implement individualized care plans, offer emotional and medical support, and ensure the best quality of life for patients. This role includes after-hours phone and home visit coverage to support symptom relief and end-of-life care.
Key Responsibilities
- Provide medical and emotional support to pediatric patients and families.
- Develop and implement individualized care plans with the interdisciplinary team (IDG).
- Conduct assessments, coordinate services, and ensure appropriate care levels.
- Educate families and facility staff on pediatric hospice and palliative care.
- Supervise LPNs and Hospice Aides as needed.
- Manage medical equipment, supplies, and medications.
- Participate in after-hours phone triage and home visits as assigned.
- Collaborate with community resources and facilitate referrals.
- Maintain compliance with CMS, state regulations, and nursing best practices.
