Our hospice care team is seeking a skilled and compassionate RN to fill the role of Patient Care Manager. This full-time salaried position requires 2-3 years of leadership experience and a strong understanding of nursing practices.
The ideal candidate will possess at least 4 years of full-time experience as an RN or equivalent and be responsible for ensuring the delivery of safe and efficient patient care, adhering to relevant standards of accepted nursing, regulatory, medical practice, and payer source criteria.
Key responsibilities include compliance with hospice policies and procedures, promotion of quality care and safety of patients and staff, and provision of in-services to staff to promote health and safety of patients and familiarize staff with regulatory issues and agency policy and procedures.
Additionally, the successful candidate will engage in thorough problem resolution and complaint investigation, and provide educational support to staff to address identified needs.
