JOB SUMMARY

This is a hybrid position for palliative care and hospice. The Advanced Practice Clinician will collaborate with Physicians in providing Palliative Care to patients in St. Luke’s Hospital and St. Luke’s Clinics, as well as working within the Hospice program. Approximately 75% of the provider's time will be spent with a hospital based palliative care team. Responsibilities will include managing new consults and follow up visits as determined by group collaboration. Ideally the NP will manage approximately 1/3 of the total referrals. Serve as a resource for the RN team members and hospital staff. Potential for providing education re palliative care and hospice to wider hospital and clinic communities of St Luke's.
Approximately 25% of the provider's time will be spent supporting the outpatient and community based hospice program of St Luke's. Responsibilities will include but not be limited to providing face to face visits, attending IDG and actively participating in treatment decisions and supporting nursing staff on the inpatient unit. Provider will provide some primary patient management once patients are enrolled in hospice, primarily regarding medication management.
As staffing continues to evolve, the percentage spent with any specific division of the Hospice and Palliative care team may change as well.

MINIMUM QUALIFICATIONS

Education: See Licensure/Certification/Registration requirements.Experience: At least one (1) year experience in either ambulatory or primary care setting.Licensure/Certification/Registration: Must be licensed as an Advanced Practice Clinician (Nurse Practitioner or Physicians Assistant) with the State of Minnesota. Obtain Wisconsin licensure within twelve (12) months of hire into position. BLS Certification with a designation of either BLS Provider or Healthcare Provider from either the American Heart Association or American Red Cross or Military Training Network. Wisconsin Background Study upon hire and renewed per Wisconsin state law.

Additional Information
This is an exciting, new position open to a nurse practitioner with experience working in hospice and palliative care. Position is open to 0.8-1.0 FTE and will include 3-4 days/week in the hospital and 1 day per week in hospice. The qualified candidate will be skilled at developing the practice model and be able to work autonomously with the support of the hospice director and nurses.
