Job Title: Registered Nurse
Hospice of Chenango County requires new employees to have the Covid vaccine, we will provide vaccinations to new hires if needed.
Reports To: Director of Patient Programs
FLSA Status: Non-Exempt
Summary: RN Case Manager for Chenango County
Responsible for the delivery of patient care by performing the following duties.
Duties and Responsibilities include the following. Other duties may be assigned.
- Responsible for being a subject matter expert in death, dying, and grief.*
- Responsible for providing quality end-of-life care striving for a 90% benchmark on all data points.*
- Demonstrates compassion in all interactions representing the agency.*
- Maintains professionalism in all interactions.*
- Responsible for the delivery of patient care through the fundamental nursing process of assessment, planning, implementation, and evaluation using established guidelines.*
- Functions as a member of the interdisciplinary team and under general supervision and direction of the patient and family care coordinator.*
- Assumes primary responsibility for patient and family caseload which includes assessing, planning, implementing, and evaluation of phases of the nursing process.*
- Acquires and maintains current knowledge in hospice nursing practice.*
- Meets minimum productivity standards.*
- Completes initial comprehensive assessment within five days of new admission in a thorough manner.*
- Obtains data on physical, psychological, environmental, social, and spiritual factors that may influence patient and family health care status and communicates information to appropriate members of the interdisciplinary team for incorporation into the plan of care.*
- Initiates communication with attending physicians, Hospice medical director, and other agencies as needed to coordinate optimal care and use of resources for the patient and family.*
- Identifies expected outcomes individualized to the patient and family plan of care.*
- Develops nursing plan of care that prescribes interventions to attain expected outcomes.*
- Implements interventions identified in the plan of care.*
- Evaluates patient and family progress toward attainment of goals.*
- Demonstrates high standard of nursing practice related to end-of-life care and pain and symptom management through competency evaluations.*
- Provides emotional support to the patient and family and assists with patient and family coping throughout the disease process.*
- Maintains current patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.*
- Responsible for ensuring documentation is concise, accurate, and timely.*
- Shares in providing 24-hour seven day a week on-call coverage as directed.*
- Provides appropriate support at the time of death.*
- Maintains professional boundaries at all times.*
- Maintains regular communication with the attending physician concerning patient and family care needs and responses to interventions.*
- Collaborates and communicates with other professionals including PT, OT, dietician, and others remembering patient and family needs and plan of care.*
- Meets regularly with the interdisciplinary team members to review concerns or complex issues, shares professional support, and optimizes communication to enhance the quality of patient care and professional growth.*
- Attends all mandatory in-services on an annual basis.*
- Orientates LPNs to patient plan of care and supervises delivery of care.*
- Contributes to the orientation of new staff.*
- Assists other team members as needed.*
- Supports the organization’s mission, vision, and values.*
- Demonstrates a commitment to the hospice philosophy of care.*
- Demonstrates sensitivity to and understanding of death, dying, and bereavement.*
- Demonstrates ethical conduct and follows practices outlined in the corporate compliance plan.*
- Demonstrates knowledge and understanding of agency policy and procedures outlined in the employee handbook.*
- Complies with the health and safety requirements of federal, state, and local laws.*
- Attends and participates in continuing education for professional development.*
- Respects and protects the confidentiality of all agency information.*
- Maintains flexibility in assignments to meet agency needs and accepts other duties as appropriate.*
Certificates and Licenses:
- Currently licensed to practice nursing in the state of New York
- CHPN certification required after two years of employment
- Valid New York State driver’s license

- Oxygen concentrator
- Catheter- foley and straight
- Hoyer lift
Knowledge, Skills and Other Abilities:
- Knowledge of and belief in the hospice philosophy of care
- Demonstrated ability in the management of pain and symptom control
- Ability to relate well to the needs of the terminally ill and their families
- Ability to understand, relate, and promote effective interaction between the Hospice Interdisciplinary Group and between all other hospice staff and volunteers
- Ability to communicate effectively with other physicians and health care professionals to ensure that patients experience medical care which reflects to hospice philosophy
- Strong interpersonal and public relations skills for effective leadership of all aspects of hospice medical services programming with patients, families, and the community
- Administrative ability in the areas of planning, organization, coordination, and supervision
- Experience in public speaking and teaching
- Proficiency in the use of computers and clinical computer software
- Willingness to travel to homes of patients and families and to provide own transportation
