AuthoraCare Collective is currently seeking a Physical Therapist Assistant (PTA) Case Manager Homecare / Long-term Care position for ourIntegrated Health Services (Skilled home-based-care / home health) department supporting our High Point, Kernersville and Winston-Salem service areas. This position is Full-Time : Monday - Friday 8 : 00am - 5 : 00pm. Must be able to travel to, enter, and function in the homes of any patient / caregiver within the service area.
The Physical Therapist Assistant provides services to retore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease under the direction of the Physical Therapist. Physical Therapist Assistant adhere to the therapy Plan of Treatment for a case load of home health patients in collaboration with other members of the interdisciplinary team (IDT). Physical Therapist Assistant works with the Physical Therapist and patient physician to maintain a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance.
Physical Therapist Assistant establish and maintain therapeutic relationship with patient, including providing community support for resources and education. Will dcument the goals and outcomes of care. Requirements
Education and Experience :
- Associate degree in Physical Therapy from an American Physical Therapy Association.
- Minimum of one-year physical therapy assistant experience home health or facility preferred.
- Current and unrestricted Physical Therapy Assistant Licensure.
- Current CPR certification.
- Other : Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
