SUMMARY: The Clinical Intake Representative is responsible for managing all intake processes required to efficiently process referrals for admission to Hospice Savannah, Inc. services. This includes reviewing all clinical records for eligibility, reviewing payer sources, obtaining all pre-certifications, managing incoming calls, scheduling visits and education patients and families on services within Hospice Savannah, INC When appropriate. Qualifications
QUALIFICATIONS: Graduate of an accredited school of Nursing, Licensed Practical Nurse license required with experience in hospice, oncology, or home health preferred. Knowledge of Joint Commission standards, state and federal regulations regarding care and reimbursement required with hospice specific knowledge preferred. CPR certification required; Certified Instructor preferred. Previous experience in clinical education, preparing and planning educational in-services preferred. Valid driver's license, automobile Insurance and reliable transportation required. Excellent communication, customer service, leadership and critical thinking skills required to include conflict resolution and the ability to motivate others required. Computer proficiency in Microsoft Office and database knowledge required.
PHYSICAL DEMANDS: Position requires moderate to intense visual concentration, reading, using a computer and reviewing patient charts. This position has daily contact with patients, families, medical facilities, physicians, physician office staff, vendors/suppliers and HSI staff requiring excellent communications and problem solving skills and professional demeanor. This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The Office Nurse requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 pounds using proper body mechanics.
WORK ENVIRONMENT: This position works a 30-40 hour work week, to include rotating days, evenings, or weekends and occasional requests to work additional days/hours/holidays as needed. This position may require some commuting between the Hospice Savannah facilities utilizing a personal vehicle. This position encounters stressful situations with patients and their families regarding end of life care. The Office Nurse may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations (potential or actual). The Clinical Intake Representative must be able to accurately, timely and concisely document the patient/family encounter using a computer.
