Nurse Practitioner - Position Summary

The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. This role involves providing Hospice/Palliative care through the evaluation and assessment of health history, health promotion, diagnosis of medical conditions, management of health problems, and coordination with patients, families, and healthcare practitioners. Responsibilities include conducting physical assessments, performing face-to-face assessments, and evaluating Hospice/Palliative eligibility in consultation with the interdisciplinary team. The Nurse Practitioner works within the community to plan and direct services that meet the needs of individuals and families in their homes and communities.

Accountability

The Nurse Practitioner reports to the Medical Director.

Essential Duties and Responsibilities

Patient Care

  • Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act under physician supervision.
  • Conducts history and physical examinations, evaluates patient conditions, and documents findings in medical records.
  • Develops individualized plans of care, prescribes medications/treatments, and coordinates Hospice/Palliative care services.
  • Completes face-to-face visit requirements prior to third and subsequent Hospice/Palliative benefit periods to assess eligibility.
  • Reviews clinical information and oversees patient care in collaboration with the interdisciplinary team.
  • Ensures appropriate symptom control measures through interdisciplinary collaboration.
  • Develops treatment protocols and proposes intervention options based on clinical literature.
  • Consults with the supervising physician regarding care outside standard practices and protocols.

Communication

  • Collaborates with the interdisciplinary team to create, review, and revise the patient’s plan of care.
  • Consults with attending physicians, hospice medical directors, and other healthcare practitioners regarding patient eligibility, care needs, and medical management.
  • Coordinates care with community health practitioners.
  • Participates in hospice interdisciplinary group meetings.
  • Completes and submits accurate clinical documentation, including physical examinations, assessment notes, medical orders, and collaboration notes.
  • Provides education and counseling to patients, families, and caregivers regarding medical, psychological, and spiritual concerns.
  • Ensures a safe environment for patients and continuity of care.
  • Establishes and nurtures relationships with staff and community referral sources to support program growth.

Additional Duties

  • Maintains compliance with Medicare/Medicaid, state/federal regulations for Hospice/Palliative care services.
  • Adheres to Medicare conditions of participation and state healthcare regulations.
  • Complies with HIPAA regulations in accordance with federal, state, and organizational policies.
  • Participates in quality monitoring and improvement initiatives.
  • Pursues professional growth through continuing education and training.
  • Demonstrates expertise in communication and counseling for end-of-life care.

Minimum Qualifications

  • Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
  • Current nursing licensure and CPR certification.
  • Master’s degree or equivalent experience with at least one (1) year Nurse Practitioner experience and one (1) year hospice or palliative care experience.
  • Accreditation from an approved certifying body for advanced practice nursing as required by state regulations.
  • Preferred certification in Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics.
  • Strong observation, verbal and written communication, problem-solving, and nursing skills.
  • Valid driver’s license with an insured and reliable automobile.

Physical Requirements

  • Constant (over 66% of time) activities include sitting, standing, walking, talking, using hands, and observing. Requires visual acuity and hearing for nursing tasks.
  • Frequent (34%-66% of time) activities include reaching, stooping, bending, kneeling, crouching, and lifting up to 25 pounds. Requires assisting patients with movement, driving in various weather conditions, and navigating uneven terrain.
