What You’ll Do:
✨ Serve as the key point of contact for patients, caregivers, and providers, ensuring a smooth palliative care experience.
✨ Coordinate and schedule bi-weekly Interdisciplinary Team Meetings to ensure top-notch patient care.
✨ Guide patients and families through the admission process, ensuring they understand services and receive the support they need.
✨ Work closely with social workers to help patients access vital resources and healthcare services.
✨ Collaborate with healthcare professionals to enhance patient care and improve outcomes.
✨ Maintain compliance by ensuring documentation and billing processes are completed accurately and on time.
✨ Support program outreach by working with our Community Liaison to engage providers, community organizations, and the public.
✨ Play a lead role in program development, quality assurance, and continuous improvement efforts.
Who You Are:
✅ A compassionate professional with 2-3 years of health or social services experience (Medical Assistant, CNA, or LVN preferred).
✅ Tech-savvy and comfortable with electronic medical records and Office 365.
✅ A clear and empathetic communicator, both verbally and in writing.
✅ Organized, proactive, and able to juggle multiple tasks with ease.
✅ Respectful of diverse lifestyles, cultures, and values, always maintaining a non-judgmental approach.
✅ Knowledgeable about palliative care philosophy and dedicated to patient-centered care.
✅ A licensed driver with a reliable, insured vehicle.
