Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that patients receive the necessary medical services while promoting optimal health outcomes. This position requires strong clinical skills, excellent communication abilities, and a commitment to patient advocacy across various settings including hospital medicine, outpatient care, and managed care environments.
Responsibilities
- Conduct comprehensive patient assessments to determine individual healthcare needs.
- Develop and implement personalized care plans in collaboration with interdisciplinary teams.
- Monitor vital signs and evaluate patient progress throughout treatment.
- Provide education and support to patients and their families regarding medical conditions and treatment options.
- Coordinate care transitions between different healthcare settings, including hospital, outpatient, and home care.
- Advocate for patients' needs within the healthcare system to ensure access to appropriate services.
- Maintain accurate documentation of patient interactions and care plans in compliance with regulatory standards.
- Participate in quality improvement initiatives aimed at enhancing patient care outcomes.
Qualifications
- Current Registered Nurse (RN) license in good standing.
- Experience in hospital medicine, critical care, or medical-surgical nursing preferred.
- Familiarity with managed care practices and patient assessment protocols.
- Experience working with geriatric populations is advantageous.
- Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals.
- Ability to work independently as well as part of a collaborative team environment.
- Proficiency in electronic health record (EHR) systems for documentation purposes.
- Hospice and / or Home health care experience preferred
