What are the responsibilities and job description for the PRN RN Triage position at Hospice?
Weekday night shift 12a-8:30a and weekend hours 8am-8:30pm. All counties within service area.
Uses the nursing process and care management strategies to coordinate care and provide nursing services for hospice patients and their families in the home, long term care facility, or assisted living facility.
Primary tasks, duties, and responsibilities:
1. Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team.
2. Under the direction of a licensed physician, performs patient assessment, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
Educational/Professional:
1. Current Florida Licensure as RN. BSN or MSN degree preferred.
2. Minimum one year recent acute care experience in a Hospital or Home Health environment
