Care Manager

This role is essential in many of the aspects of patient care, including helping patients and their families navigate the medical system, assessing and monitoring patients' and family members' mental and emotional health, providing short-term counseling and therapy, and communicating patient needs and concerns to the larger medical team. The Care Manager also completes proactive planning for coordinating transition of care to discharge with post-acute service assistance as well as offering community services to patients in need.

Education

Bachelor's Degree in Social Work is required.

Master's Degree in Social Work is preferred.

Experience

Prior experience in social work or past clinical social service experience, directly transferrable to a healthcare environment, is preferred.

Certifications, Licenses and Registrations

Must be licensed in the state of practice or meet state requirements for social work in a health care setting within 6 months of start date.

Illinois: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LSCW) is required.

Wisconsin: Certified Social Worker (CSW), Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW) or Licensed Clinical Social Worker (LSCW) is required. Basic Life Support Certification (BLS) is required within 60 days of hire.
