What are the responsibilities and job description for the Social Worker-LCSW-Internal Medicine position at Hospital Sisters Health System?
HSHS Medical Group is seeking a Social Worker to join our team at Internal Medicine Clinic. Ideal candidates are patient focused, mission driven individuals who will be responsible for providing therapeutic services to patients dealing with a wide range of issues by counseling them and helping patients and their families navigate the medical system, assessing and monitoring patients' such as depression, anxiety, and eating disorders by counseling them on managing ongoing health conditions and making positive behavior changes.
Responsibilities include :
- Meeting with patients to assess their histories, condition, and needs.
- Interviewing family members when needed.
- Providing and communicating recommendations.
- Collaborating with provider(s) to produce a treatment plan that best meets the needs of the patient.
- Monitoring patients' progress
Position Specifics :
o Schedule : Full-time; 40 hours / week; Monday-Friday; 8am-4 : 30pm
o Location : Effingham, IL

Pay Range : 28.14 - $42.21
A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
